Asthma Treatment Service - Terms and conditions

What the service is

1. For a fixed monthly charge this service will provide the following:

a. Asthma advice and guidance via the telephone, your online patient record or Skype, between 0830 – 1800hrs Monday to Friday (or such as hours amended from time to time).

b. The regular supply of asthma medicines.

c. One aero-chamber device with the first payment.

The first charge will occur when you sign up to service, and then repeat on a monthly basis on or around the same day each month.

2. Your regular supply of asthma medicines depends on what plan you are on:

a. Plan 1 means: the supply of one Clenil 100mcg inhaler and one Ventolin inhaler at sign up and then the further supply if requested of one Clenil 100mcg inhaler every 50 days and one Ventolin inhaler every two months. The total maximum supply of medicine per 12 month period with this plan is eight Clenil inhalers and six Ventolin inhalers.

b. Plan 2 means: the supply of one Clenil 200mcg inhaler and one Ventolin inhaler at sign up and then the further supply if requested of one Clenil 200mcg inhaler every 50 days and one Ventolin inhaler every two months. The total maximum supply of medicine per 12 month period with this plan is eight Clenil inhalers and six Ventolin inhalers.

c. Plan 3 means: the supply of one Seretide 50mcg inhaler and one Ventolin inhaler at sign up and then the further supply if requested of one Seretide 50mcg inhaler every month and one Ventolin inhaler every two months. The total maximum supply of medicine per 12 month period with this plan is 12 Seretide inhalers and six Ventolin inhalers.

d. Plan 4 means: the supply of one Seretide 125mcg inhaler and one Ventolin inhaler at sign up and then the further supply if requested of one Seretide 125mcg inhaler every month and one Ventolin inhaler every two months. The total maximum supply of medicine per 12 month period with this plan is 12 Seretide inhalers and six Ventolin inhalers.

e. Plan 5 means: the supply of two Ventolin inhalers at sign up and then the further supply if requested of one Ventolin inhaler every three months. The total maximum supply of medicine per 12 month period with this plan is six Ventolin inhalers.

Additional charges

3. The fixed monthly fee assumes collection of the medicine from any participating LloydsPharmacy. If you wish to have the medicine delivered to you by post then you will need to pay the appropriate delivery charge each time.

Cancellations and refunds

4. The service is provided for a minimum of two months. If you cancel within two months we will charge you a cancellation processing fee of £15.00.

5. After two months we require one month's notice to cancel the service.

Our clinical responsibilities

6. We will carry out our clinical responsibilities to the best of our abilities and in a responsible way. Where there is disagreement on clinical matters our decision is final.

7. The service will be provided by a doctor or such person designated by the doctor as competent to carry out that role.

8. At any point we may decide, based on our clinical judgment, that it is not clinically safe for you to have your asthma managed through this remote service. In such case we can cancel the monthly fee and stop providing the service.

9. At any point we may recommend, based on our clinical judgment, that you change plan. You do not have to accept this change - accepting may involve a higher or lower monthly fee - but we reserve the right to cancel the monthly fee and stop providing the service.
